Position: Practical Nurse Role with Carefor
Become a Registered Practical Nurse with Carefor, Eastern Ontario’s leading home care provider. In this role, your communication skills and nursing strategies will be vital for delivering quality client care.

You will work 45 hours bi-weekly, engaging directly with clients to implement personalized care plans while collaborating with healthcare teams. Your responsibility is to assess and document client needs accurately, ensuring that the care provided aligns with established protocols. Your knowledge in nursing will support client independence and health education for families.

Key Responsibilities:

• Conduct client and family interviews to gather information

• Assess client needs and the home environment thoroughly

• Update client care plans in collaboration with the healthcare team

• Document assessment findings and care plans clearly

• Provide health teaching based on adult learning principles

Requirements:

• Registered Practical Nurse with the Ontario College of Nurses

• Experience in clinical practice is an asset

• Background in community health preferred

• Excellent communication and organizational skills

• Adherence to nursing standards and practices
