Key Responsibilities:
• Deliver primary care onsite to seniors
• Design person-centered care plans for patients
• Collaborate on clinical policies and guidelines
• Conduct monthly Chronic Care Management visits
• Facilitate team huddles for improved coordination
Requirements:
• Nurse Practitioner with geriatrics experience preferred
• Flexible to travel in the San Francisco Peninsula
• Strong communication and leadership skills
• Able to adapt to a fast-paced setting
• Experience with tech-enabled care solutions
