In this full-time or part-time position, your responsibilities include conducting thorough client assessments, creating individualized care plans, and coordinating with community support services. Key Responsibilities:
• Assess clients and implement personalized care plans
• Act as the primary nursing lead for client care
• Connect clients with community support services
• Monitor and evaluate care effectiveness based on client needs
• Collaborate with multi-disciplinary health teams for optimal outcomes
Requirements:
• Must hold a current license from the College of Nurses Ontario
• Canadian Nurses Association certification is advantageous
• Strong organizational and communication skills required
• Valid driver’s license and vehicle necessary for travel
• Current CPR and immunization records required
