As a Nurse Practitioner with The Ottawa Hospital, you will be instrumental in enhancing healthcare delivery for complex older adults. This role involves delivering evidence-based care while supporting transitions between hospitals and long-term care. Key responsibilities include mobile health service delivery, working closely with medical teams, and actively participating in capacity building initiatives in LTC settings.
Key Responsibilities:
• Deliver urgent mobile nursing services in LTC homes
• Address health concerns to minimize hospital admissions
• Enhance system flow between LTC and hospitals
• Collaborate effectively with healthcare team members
• Actively participate in capacity building efforts
Requirements:
• Bachelor of Science in Nursing;
Master’s preferred
• Minimum of two years clinical experience
• Nurse Practitioner Certificate and Registered Nurse Extended Class
• Knowledge of acute and geriatric care principles
• Current driver's license and eligibility to work in Canada
