Nurse Practitioner, Emergency Outreach Pilot

Position Summary

This position is for the Nurse Led Outreach Team (NLOT) and will be based at the Civic campus. Local/Regional travel required. Join NLOT and play a vital role in improving the lives of seniors by delivering exceptional, evidence‑based care. You will deliver urgent mobile nursing services to residents in LTC homes, addressing emerging health concerns and reducing unplanned transfers to emergency departments and hospital admissions;

support capacity building within LTC and support system flow between hospitals and LTC environments.

Days, 8 hour shifts.

• Bachelor of Science in Nursing
• Masters degree in Nursing or a related discipline
• Member of Registered Nurses Association of Ontario
• Member of Nurse Practitioners Association of Ontario
• Minimum of two (2) years recent clinical experience in an acute care setting working with medically complex older adults with strong skills in autonomous decision making
• Nurse Practitioner Certificate
• Registered Nurse Extended Class (EC)
• Demonstrated ability to respond appropriately to rapidly changing clinical conditions
• Demonstrates current understanding of acute and geriatric principles
• Demonstrates knowledge of principles and issues related to the delivery of acute episodic care in LTC setting
• Demonstrates a commitment to continuing education
• Demonstrates experience and willingness to actively participate in building capacity in the LTC setting
• Expertise in advanced health assessment, diagnosis and clinical procedures
• Demonstrates ability to work collaboratively and communicate (in oral and written formats) effectively as an active member of the health care team
• Experience with word processing, spreadsheet and database applications
• Current driver's license
• Must be eligible to work in Canada

• Experience in an acute setting where strong skills in autonomous decision making have been developed (e.g., Emergency Department, Northern or underserved area)
• Post Basic Certificate in Gerontology
• Experience working in the LTC setting
• Proficiency in English and French oral expression (level 4 - advanced) and comprehension (level 4 - advanced)

The Ottawa Hospital uses a 5‑level language proficiency scale to define the required level of language skills in English and in French for positions at the Hospital, and to evaluate job candidate language skills. Language proficiency requirements are based on the nature of the position and the level of interaction with French and English‑speaking clients.
