As an NP, you'll work under the Manager of Supportive Housing and Substance Use Health, providing essential healthcare for clients facing complex health issues. Your responsibilities include health assessments, diagnoses, and collaborations to enhance patient care within a culturally relevant framework. Emphasizing risk management and emotional support, you will address diverse health needs in non-traditional environments.
Key Responsibilities:
• Deliver comprehensive medical assessments and triage services
• Implement health promotion and disease prevention strategies
• Work closely with interprofessional teams for coordinated care
• Continuously evaluate and monitor client health programs
• Manage a progressive roster of clients effectively
Requirements:
• MScN RN(EC) or equivalent with extended registration
• Minimum two years’ nurse practitioner experience
• Strong skills in engaging diverse communities
• Familiarity with EMR systems and harm reduction models
• Excellent communication and interpersonal skills
