As a Full-Time Nurse Practitioner, you'll provide onsite primary care services in senior living communities. Engage directly with residents, develop tailored care plans, and leverage the strengths of a multi-disciplinary team. This position fosters a collaborative environment designed to enhance the well-being of seniors while achieving comprehensive health goals.
Key Responsibilities:
• Provide primary care services to older adults
• Create and implement detailed care plans
• Work with Medical Director on relevant policies
• Conduct monthly Chronic Care Management sessions
• Lead and inspire the clinical team during huddles
Requirements:
• Nurse Practitioner preferably with geriatrics experience
• Willing to travel within the San Francisco Peninsula
• Positive, adaptable mindset in dynamic settings
• Excellent verbal and written communication skills
• Familiar with technology for patient care management
