In this role, you will be responsible for performing detailed participant assessments and devising strategic care transition plans in collaboration with families and healthcare teams. Candidates need a minimum of three years of experience in managing medically complex populations. Your efforts will ensure continuity of care and support a dignified living environment for participants.
Key Responsibilities:
• Conduct thorough evaluations of hospitalized participants
• Coordinate inpatient care and discharge planning
• Develop individualized transition care plans
• Work with interdisciplinary teams for care continuity
• Educate participants about their treatment plans
Requirements:
• Unrestricted RN license in California
• Minimum 3 years experience with older adults
• Current CPR/BLS certification necessary
• Valid driver’s license required
• Preferred:
Alzheimer’s Certification
