As a valued member of the interdisciplinary team, the Registered Practical Nurse will coordinate and deliver patient care, assessing needs and implementing medical plans. Your responsibilities will directly impact patient outcomes and support the commitment to healthcare excellence. Working with HPHA, you will engage with community partners while enhancing your skills and knowledge in diverse clinical settings.
Key Responsibilities:
• Assess and identify patient needs for effective care
• Implement and monitor patients' medical plans
• Administer medications and IV therapy with care
• Provide education and support to patients and families
• Collaborate with interdisciplinary teams for optimal care delivery
Requirements:
• Degree/diploma from a recognized institution
• Current registration with the College of Nurses of Ontario
• Basic Life Support certification
• Two years nursing experience preferred
• Strong communication and organizational skills
