This permanent position involves managing clinical operations in Transitional Care under the Director of Clinical Programs. You will leverage your leadership and organizational skills to deliver patient-centered care while guiding a multi-disciplinary team, including nurses and allied health professionals. Key Responsibilities:

• Oversee daily operations of the Transitional Care unit

• Manage patient care activities in accordance with legal standards

• Collaborate closely with leadership for unit coverage and patient flow

• Address clinical issues and implement best practices

• Support staff development through training and mentorship

Requirements:

• Undergraduate degree in Nursing or Allied Health

• Minimum 2 years in Medical or Surgical Nursing

• At least 2 years in a leadership role

• Professional practice qualification in Ontario

• Strong management and communication skills
