Community Health Nurse Practitioner Job

Join Pinecrest-Queensway Community Health Centre in Ottawa as a Community Health Nurse Practitioner. This part-time role emphasizes patient autonomy and collaborative care within a multi-disciplinary team focused on falls prevention.

In a position that requires 21 hours a week, you will work with diverse client populations, managing their primary health needs. Key Responsibilities:

• Independently manage a dedicated panel of community clients

• Diagnose and treat various health conditions for all ages

• Develop and implement comprehensive care plans

• Engage in community health education on prevention tactics

Requirements:

• Master’s in Nursing and CNO Extended Class registration

• At least 2 years of Nurse Practitioner experience

• Strong background in community health and teamwork

• Ability to work flexible hours and possess a vehicle

• Bilingualism in English/French is an asset
