As a Nurse Case Manager, you will play a vital role in providing consultations, health teaching, and nursing assessments. With a focus on community, clinic, and hospital settings, your experience will help develop and implement impactful health services. You will collaborate with stakeholders and assist in monitoring compliance and engagement throughout treatment programs.
Key Responsibilities:
• Provide virtual nursing assessments and health recommendations
• Assist in program development and service delivery
• Support the creation of training materials and service standards
• Monitor client progress and engagement in treatment
• Document interactions and provide reporting updates
Requirements:
• Must be RN, RPN, or LPN
• Minimum 3 years of nursing experience
• Registered and in good standing with Provincial College
• Proficiency in English for effective communication
• Strong critical thinking and organizational skills
