As a part of the Integrated Health Services team at Centretown CHC, you will provide primary health care while collaborating with a team of healthcare experts. Your experience as a Nurse Practitioner will enable you to address a variety of health issues, including complex mental health scenarios. You will provide important health services in a community health setting focused on accessibility and inclusion for all.

Key Responsibilities:

• Offer primary health care in a community center

• Engage in chronic disease management for patients

• Facilitate same day access clinics and client support

• Implement a health equity and anti-oppression approach

• Guide medical learners and support their training

Requirements:

• Master’s degree in Nursing required

• Minimum of two years experience in community care

• Certified with the College of Nurses of Ontario

• Strong verbal and written communication skills

• Availability for weekdays and rotating shifts
