This leadership role involves collaborating directly with the Director of Home & Clinical Care to set practice standards and develop innovative service delivery models. You will play a vital part in mentoring staff, analyzing care delivery, and promoting a culture of learning and accountability.
Key Responsibilities:
• Implement and evaluate practice frameworks and policies
• Mentor and coach nursing and support staff
• Lead initiatives that foster person-centered care
• Utilize evidence and analytics to enhance service delivery
• Ensure adherence to professional standards and regulations
Requirements:
• Bachelor of Science in Nursing with relevant registration
• Minimum three years’ experience in community/home care
• Knowledge of best practices in nursing and health
• Strong critical thinking and problem-solving abilities
• Bilingual in French and English is advantageous
