We are seeking a Registered Practical Nurse who is passionate about community care to join SE Health. In this role, you will conduct client assessments, develop care plans, and collaborate with a diverse care team. Key Responsibilities:
• Conduct comprehensive client assessments and develop care plans
• Act as the primary provider for a designated client caseload

Requirements:
• Current registration with the College of Nurses of Ontario
• Valid driver’s license, vehicle, and willingness to travel
• Current CPR certificate and background checks required
• Availability for days, evenings, and weekends
• Strong dedication to community care and evidence-based practice
