In this independent role, you will provide expert triage services and develop care plans in collaboration with a dedicated interdisciplinary team. Your responsibilities include conducting thorough intake assessments and maintaining detailed EHR documentation while ensuring seamless continuity of care with community providers. Additionally, you will lead health promotion initiatives and engage in quality improvement projects.
Key Responsibilities:
• Perform medical/psychiatric intake assessments
• Develop interdisciplinary care plans for patients
• Maintain accurate EHR and coordinate patient flow
• Oversee medication management and support safety
• Implement effective discharge plans and transitions
Requirements:
• NP certification in Primary Health Care or Adult
• 5 years of relevant nursing experience
• Active registration in Ontario's Extended Class
• Familiarity with healthcare legislation required
• Proven communication skills in English and French
