This full-time role emphasizes high-quality, client-centered care and encompasses comprehensive assessments and holistic planning. Candidates will work within their full scope of practice as per CNO standards and uphold AHS’s mission and values.
Key Responsibilities:
• Provide high-quality in-home client-centered care
• Conduct comprehensive client assessments for care planning
• Collaborate with the interdisciplinary healthcare team
• Adhere to infection prevention measures and best practices
• Participate in an on-call nursing rotation
Requirements:
• Current RN or RPN registration with CNO
• Proof of Covid-19 vaccination
• Valid driver’s license and vehicle
• Recent Vulnerable Sector Police Check
• Strong assessment and communication skills
