Job Summary

The Families First Case Worker for Survivors of Domestic Violence (SDV) will work with survivors of domestic violence to support them in maintaining safe and affordable housing through intensive case management support. The priority population is Survivors of Domestic Violence, along with their families who are in the process of transitioning from the City’s shelters to housing in the community. Other families who are already settled in their communities may also be a priority for the Case worker.

The SDV Housing Case Manager will provide services to women, men and children focusing on supports and resources specific to the Survivors of Domestic Violence sector consistent with the Families First model.

Job Specific Responsibilities Intake
  • Provide screening, risk assessment, and crisis intervention for families and their children who are or who have transitioned from the Shelter system to permanent housing in the community.
Engagement - Family Support
  • Within the Housing First model recognize and support the unique and challenging circumstances faced by survivors of domestic violence fleeing violence.
  • Conducting client and family interviews, engaging clients in developing personal safety plans and obtaining restraining orders if necessary.
  • Work with the families to support prevention efforts using best practices related to counseling of survivors of domestic violence and their families.
  • Make referrals for other external supports or addressing the client's needs in areas like personal safety, physical and mental health, substance abuse, housing, employment, and child care.
  • Ensure that any issues in relation to children’s safety follow meet professional ethical standards.
  • Work with other families within the Families First model as required.
Resiliency and Advocacy
  • Provide advocacy and support to assist families who have obtained permanent housing to move into the community.
  • Provide assistance and support to resolve or mediate difficulties with landlords or neighbours to maintain tenancy.
  • Intervene with individuals and families in a manner that builds skill and fosters client resiliency.
Teamwork and Collaboration with External Agencies
  • Participate in regular chart reviews and case conferences with the team as well as other service providers, as required.
  • Collaborate with other community agencies and stakeholders to share information, problem solve, identify emerging issues, analyze data and coordinate services.
  • Connect with external resources that may support client needs and establish a referral system that will streamline access and information sharing.
Administrative
  • Maintain electronic records of client and protects the confidentiality of the information.
  • Participates in data collection and analysis as required.
  • Assists in the development of procedures and guidelines for the Families First Program and in the overall planning and development of the Families First program.
  • Provides information to PQCHC staff on issues related to housing and homelessness.
  • Performs other related duties as requested.
Qualifications Education
  • Undergraduate degree in health or social science from a recognized university or an equivalent combination of education and experience.
Professional Experience
  • A minimum of three years’ experience working with Survivors of Domestic Violence issues including:
    • Knowledge of Trauma Informed Care.
    • Knowledge of supports for children who witness violence.
    • Comfortable liaising with:
      Ottawa Police Services;
      Ottawa Victim Assistance Program;
      Children Aids Society; shelters.
    • A community-based setting with an emphasis on community outreach and working with homeless or high risk populations.
    • Experience with housing placement and landlord liaison.
  • Ability to work in a team environment within PQ team as well as stakeholders within the Survivors of Domestic Violence sector.
