About the Role

Rexdale Community Health Centre is a non‑profit, multi‑service facility that offers primary health care, settlement, and community development activities for women, families, youth, adults, and seniors from diverse cultural backgrounds. The Centre is dedicated to delivering accessible and sustainable programs and services to diverse communities. It employs service delivery methods that incorporate advanced access principles, utilizing group visits and self‑management models.

Currently, the Centre is seeking a Mental Health Case Manager to collaborate with the mental health team in providing support and primary health care to Black children, youth, and mothers. The successful candidate will join the Centre’s clinical team.

Responsibilities
  • Coordinate the integration of social services/case management functions into the client care with other departments, external service organizations, agencies and healthcare facilities.
  • Acts as client advocate, investigates and reports adverse occurrences and performs staff education related to mental health.
  • Promote effective and efficient utilization of clinical resources.
  • Mobilizes resources and interviews, as needed, to achieve the expected goal to assist in achieving desired outcomes within the desired timeframe.
  • Conducts review for appropriate utilization of services from intake admission through discharge. Evaluate client satisfaction and quality of care provided.
  • Complete expanded assessment of patients and family needs at time of intake, such as the GAINS assessment.
  • Assess client’s progress on established action plan.
  • Refers cases where clients and/or family would benefit from counselling required to complete a complex discharge plan to the social worker.
  • Serve as a client advocate. Enhances a collaborative relationship to maximize the client’s and family’s ability to make informed decisions.
  • Collaborate with clinical staff in the development and execution of the plan of care and the achievement of goals.
  • Directs and participates in the development and implementation of client care policies and protocols in order to provide advice and guidance in handling special cases or client needs.
  • Coordinates the provision of social services to clients, families, and significant others to enable them to deal with the impact of illness on individual family functioning and to achieve maximum benefits from healthcare services.
  • Perform other duties and committee work as assigned.
  • Adhere to the Centre’s policies and procedures, including confidentiality, AODA, and health and safety policies.
Qualifications
  • A post-graduate degree in Social Work, Social Sciences, Health Sciences, Public Health or a combination of relevant education and at least three years’ direct experience working with 2

    SLGBTQ+ youth and communities.
  • Three to five years of experience supporting youth in community development and/or health promotion within culturally, racially, and sexually diverse communities, with demonstrated knowledge of issues impacting 2

    SLGBTQ+ populations.
  • Demonstrated ability to work independently and as part of a collaborative team, while centering the voices and leadership of 2

    SLGBTQ+ youth.
  • Experience designing, implementing, and managing projects that respond to the specific needs of 2

    SLGBTQ+ youth, including support for mental health, housing stability, education, and employment.
  • Strong skills in building relationships with youth, families, and community stakeholders to address barriers such as homophobia, transphobia, racism, and systemic inequities.
  • Excellent verbal and written communication skills, with the ability to adapt to different audiences, including youth, families, community partners, and funders.
  • Proven ability to provide leadership, consultation, and collaboration within multi-disciplinary teams, ensuring inclusive and affirming practices are upheld.
  • Skilled in program planning, facilitation, evaluation, and reporting, with a commitment to youth-led and community-driven approaches.
  • Strong advocacy, networking, and problem-solving skills to address systemic barriers impacting 2

    SLGBTQ+ youth, including housing, healthcare, education, and safety.
