Community-Focused Case Manager

You will be engaged in a coordinated approach to provide comprehensive client support, aiding individuals in navigating housing challenges and connecting with essential services. Experience in urban homelessness and ability to handle high-stress situations will be key in this role. You will develop client-driven plans while advocating for solutions in an inclusive environment.
Key Responsibilities:
• Conduct comprehensive assessments for clients’ needs
• Support clients with crisis intervention strategies
• Engage with community resources on behalf of clients
• Provide guidance for housing searches and viewings
• Document case history and maintain accurate records
Requirements:
• Community College degree in Social Work or equivalent experience
• Minimum 2 years in case management role with homeless populations
• Proficiency in motivational interviewing
• Valid driver’s license and clean abstract required
• Current clear Police Reference check for Vulnerable Sector
