Key Responsibilities:

• Coordinate hospital services with social workers for families

• Process applications for community resources and assistance

• Create and maintain resource packages and information

• Follow up on service applications and address parental concerns

• Assist during inpatient stays with transportation and meals

Requirements:

• College diploma in Social Services or equivalent

• Three years of experience with medically challenged families

• Knowledge of community resources for children with disabilities

• Strong interpersonal and organizational skills

• Bilingual in French and English required
