Key Responsibilities:

• Implement effective social work interventions

• Engage with interdisciplinary team members

• Guide discharge planning and subsequent care

• Mentor students during their practical training

• Leverage community resources for patient benefit

Requirements:

• Master’s degree in Social Work (MSW) required

• Registered with the Ontario College of Social Workers

• Experience with in a hospital environment preferred

• Knowledge of community support avenues beneficial

• Bilingual in French and English is an asset
