Palliative Care Social Worker

Become a pivotal part of CHEO's inter professional team as a Social Worker focused on palliative care. You will offer counseling and support to families facing life-limiting illnesses, both in the hospice and the community.
The Social Worker position at CHEO and Roger Neilson Children's Hospice provides essential support for children and families during crucial times. This role involves conducting psychosocial assessments and providing crisis intervention to navigate health-related emotional challenges. You will advocate for families, coordinating community resources while contributing to the continuity of care for patients.
Key Responsibilities:
• Conduct psycho-social assessments through interviewing families
• Provide individual and group counseling for emotional support
• Advocate for patients with community services
• Collaborate with the interdisciplinary team for care continuity
• Facilitate support groups for children and families
Requirements:
• M.S.W. from an accredited university program
• Member of the Ontario College of Social Workers
• Minimum 2 years' pediatric hospital experience
• Strong ability for independent assessment and documentation
• Bilingualism in English and French preferred
