Position: Social Worker for Home Health Services
Join CBI Home Health as a Social Worker in the East Ottawa region. Work to ensure patients receive optimal care and community support through effective transitions from hospital to home.

This role emphasizes your ability to address patients' psychosocial needs and helps connect them to necessary resources. You’ll collaborate with a dedicated interdisciplinary team to improve outcomes and reduce hospital readmissions while maintaining a flexible work schedule.

Key Responsibilities:

• Facilitate transitions by addressing psychosocial patient needs

• Connect individuals with necessary community services

• Collaborate with healthcare professionals for comprehensive care

• Maintain registration with Ontario's social work standards

• Travel within the community for home visits

Requirements:

• Degree in Social Work (Bachelor or Master)

• Registration with the Ontario College of Social Workers

• 1-2 years of community experience is an asset

• Must be bilingual in English and French

• Strong communication and independent working skills
