Position: Social Worker at WelbeHealth
Join Welbe Health as a Social Worker to provide vital services for frail seniors. Work collaboratively with a dedicated interdisciplinary team to promote independent living and community resource access.

In this role, you will maintain a caseload of participants while conducting assessments and delivering targeted social work interventions. Your work will involve case management, counseling, and acting as a liaison between participants and their support systems. By facilitating care transitions, you will ensure the highest standard of care is maintained.

Key Responsibilities:

• Assess psychosocial needs of senior participants

• Develop and implement effective care plans

• Provide counseling and manage cases actively

• Facilitate care conferences and team meetings

• Handle discharge planning for disenrolled participants

Requirements:

• Master’s degree in social work (MSW) required

• Minimum one year experience with elderly populations preferred

• Facilitation of care meetings experience preferred

• CPR and First Aid certification mandatory within 180 days

• Strong collaborative and communication skills
