Drive social care excellence as the Social Work Manager 'll lead and optimize social services and behavioral health programs for seniors, ensuring they thrive in their communities.
As part of the leadership team, you will manage and develop a diverse social work staff, focusing on creating effective care plans and promoting successful outcome assessments. Your leadership will encompass strategies for compliance and collaboration across all levels, aligning with Welbe Health's core values and mission.
Key Responsibilities:
• Lead hiring, training, and performance management of social team
• Oversee social work assessments to enhance service delivery
• Ensure compliance with care standards and regulations
• Partner with Behavioral Health Specialists on programming
• Conduct necessary home visits for care coordination
Requirements:
• Preferred licensure in social work
• At least one year experience with frail or elderly clients
• Strong clinical and communication skills
• Valid driver’s license and vehicle insurance required
• CPR and First Aid certification needed within first 180 days
