You will connect with the Clinical Manager to provide comprehensive social services and support for patients.
Your role involves supervising students, engaging with community resources, and contributing to effective discharge planning. A Master’s degree in Social Work and relevant membership requirements are necessary for applicant consideration.
Key Responsibilities:
• Facilitate social work interventions for patient support
• Guide students in their social work practice
• Implement effective discharge plans alongside clinical teams
• Collaborate with multidisciplinary professionals
• Assess community resource needs for enhanced patient care
Requirements:
• Master’s degree in Social Work (MSW) from accredited institution
• Valid membership with the Ontario Social Workers Association
• Experience in a hospital setting beneficial
• Bilingual in French and English preferred
• Capable of independent work and teamwork
