Community Family Resource Worker Role

Carlington Community Health Centre seeks a Community Family Resource Worker for supporting families navigating local services. This part-time role focuses on empowering families of children aged 0–18 with essential resource connections.

As a permanent position with the possibility of hours increasing, you will work closely with vulnerable families to assess their needs and connect them with community resources such as housing, daycare, and food security.

Your role is pivotal in addressing the unique challenges experienced by diverse populations, working collaboratively to foster an inclusive support system that enhances family well-being.

Key Responsibilities:

• Manage a caseload and coordinate access to resources

• Identify high-need families for tailored support

• Create tools to track client engagement and outcomes

• Liaise with community partners to enhance program awareness

• Participate in staff training and operational problem-solving

Requirements:

• Bachelor's degree in Social Work or equivalent

• 3-5 years’ direct experience with families

• Understanding of complex family dynamics

• Fluency in English and French

• Capacity to build relationships with marginalized families
