Job Summary

The Community Worker – Wrap Around Support (WAS) plays a key role in supporting community members by providing client-centred assistance. This position works collaboratively with individuals, partner organizations, and programs across our service areas to ensure clients are connected with the resources and supports that best meet their needs. The Community Worker – WAS also works closely with internal PQCHC services and external community agencies to deliver coordinated, holistic support.

This role will report to the Community House Coordinator within the Community Development Services. The candidate will be expected to work closely with community members in low-income housing communities in PQ's catchment area.

Program

Description:

The Community Worker – Wrap Around Support (WAS) provides client-centred support at the Foster Farm Family House and the Pinecrest Terrace Community House. This position will aide and assist clients in an empathetic manner to navigate city wide resources. The Community Worker will engage one-on-one with clients during food appointments, support them in registering for programming, provide information on available supports in a multitude of areas, support with client wellbeing, client-led research on computers, complete forms, and encourage and motivate clients to become self-supporting

Job Specific Responsibilities:

Community Development and Outreach:

• Effectively carry out outreach activities to community members including identification of needs and interests based on the Centre's Strategic Plan
• Respect and value the diversity of communities and individuals by working in a manner that incorporates health promotion and recognizes the social determinants of health.
• Work in a manner that preserves confidentiality and seeks to minimize risk in keeping with PQ's Privacy and Confidentiality Policies and Procedures.
• Provide linkages with broader community resource base
• Assist clients in developing self-sufficiency and resiliency to meet their own needs by providing available options and resources.
• Provide practical assistance and internal and external referral services to clients as appropriate
• Create a supportive environment for community members to address barriers they may be experiencing
• Foster ongoing community engagement and capacity-building
• Build and maintain pertinent community partnerships in conjunction with their supervisor.
• Promote/advertise and inform the community on relevant programs and services.
• Implement conflict prevention strategies (such as de-escalation and responding to individual/community-level crises) within the community with the support of the Supervisor and mandatory training, as appropriate.

Program Development:

• Assist in developing and implementing opportunities, programs, special events and services with their respective Supervisor to ensure alignment of the mission, values, and vision of PQ
• Inform the development of program proposals, which include a statement of purpose, timelines, budget, activities, supplying inventory, etc.
