As a Family Support Resource Worker (FSRW) at Carlington Community Health Centre, you’ll work with families raising children aged 0–18, providing essential support and resource navigation. You'll create tailored action plans, connect families to vital services such as housing and benefits, and help enhance their overall well-being through integrated support. Monitoring and liaising with local partners will also be crucial to address service gaps.

Key Responsibilities:

• Provide comprehensive family support and needs assessment

• Coordinate resource access for clients in need

• Deliver programs based on community-driven assessments

• Actively link families to services and opportunities

• Develop materials for family engagement and outcomes

Requirements:

• Bachelor’s degree in Social Work or related field

• 3–5 years of experience in family support services

• Familiarity with diverse cultural communities

• Knowledge of child development and family issues

• Fluent in English and French; additional languages valued
