Position: Experienced Social Service Worker Needed
Enhance the lives of elderly residents with Bruyère Health as a dedicated Social Service Worker. This full-time, on-site role offers a chance to blend compassion with professional practice.

You will report to the Manager of Resident Experience, delivering essential social services by assessing and addressing the individual needs of our residents. This position involves working closely with families to navigate the emotional challenges of aging, all while fostering a supportive environment that prioritizes each resident’s dignity and quality of life.

Key Responsibilities:

• Support residents during their collaborative living journey

• Facilitate essential family advocacy during transitions

• Enhance overall well-being and respect residents’ individuality

• Coordinate and lead care conferences for effective communication

• Identify community services to support long-term care

Requirements:

• Two-year diploma in Social Service Work is essential

• Registration with the Ontario College of Social Workers required

• Relevant experience with seniors and palliative care

• Bilingual skills in French and English are a must

• Knowledge of medical terminology and community resources
