Safe Life Home Health Care is seeking a skilled Home Health RN Case Manager with a minimum of 1 year home health experience and proficiency in Well Sky Kinnser EMR to join our 4‑star team. Prioritize patient health and help prevent hospitalization while providing exceptional care in the comfort of patients' homes. Safe Life utilizes Andy AI, which assists our clinicians with OASIS completion and submission within 24 hours.

• Provide comprehensive nursing care to patients in their homes.
• Complete OASIS assessments and related documentation in a timely manner.
• Coordinate care plans with interdisciplinary team members.
• Conduct patient and family education on health conditions and treatments.
• Monitor and evaluate patient progress, adjusting care as needed.
• Ensure compliance with all regulatory and safety standards.
• Advocate for patients to prevent unnecessary hospitalizations.

• Valid Illinois RN license
• Valid CPR Certification
• Valid Driver's License
• Valid Auto Insurance
• Minimum 1 year home health experience
• Proficiency in Well Sky Kinnser EMR

$45 to $50 per hour OR $80,000.00 - $ per year

Monday to Friday, with weekends as needed. Expected hours: 40 to 50 per week (less for part-time).

Minimum 1‑year Home Health Experience required.
