We provide a wide range of home‑ and community‑based services, including behavioral health, substance use treatment, case management, and hourly and residential support. • Assist individuals with daily living tasks such as hygiene, cooking, cleaning, and errands
• Support clients in achieving personal goals and participating in community activities
• Provide transportation to appointments and events
• Follow and document progress on Person Centered Service Plans (PCSPs)
• Maintain clear communication with supervisors and team members

• High school diploma or equivalent
• Valid driver’s license, clean driving record, and proof of auto insurance
• Flexibility to work varied shifts and fill in as needed
