Position SummarySaint Luke’s Health System is seeking a dedicated and compassionate Palliative Care Nurse to join our team at Saint Luke’s South Hospital. This full-time position offers a critical opportunity to support patients and their families during vulnerable times, focusing on expert symptom management and facilitating essential goals‑of‑care discussions.
The successful candidate will work collaboratively within a multidisciplinary palliative care team, providing specialized support to medical professionals managing hospitalized inpatients who require comprehensive comfort care measures.
ScheduleFull time, Monday–Friday, 8:00 AM – 4:30 PM. No weekends or holidays required.
Key Responsibilities- Managing incoming referrals and palliative care consult requests.
- Collaborating effectively with diverse interdisciplinary teams, including hospital staff, providers, post‑acute care teams, and the inpatient Palliative Care team.
- Triaging patient needs to ensure timely and appropriate care delivery.
- Facilitating comprehensive goals‑of‑care conversations and connecting patients/families with necessary resources.
- Providing expert education to staff, physicians, patients, and families regarding palliative and end‑of‑life care protocols.
- Coordinating smooth transitions to subsequent levels of care, working closely with St. Luke’s Hospice and Hospice House staff.
- Collecting, reporting, and monitoring key performance indicators (KPIs) for Palliative Care services.
- Serving as the recognized subject matter expert for palliative and end‑of‑life care within the inpatient setting.
Must possess current Registered Nurse (RN) licenses in both Missouri and Kansas.
Job Requirements- 3-5 years applicable experience.
- Basic Life Support – American Heart Association or Red Cross, Registered Nurse – Various.
- Bachelor’s Degree.
