What You'll Do
  • Deliver skilled, holistic nursing care for end-of-life patients at home
  • Educate and empower families through the hospice journey
  • Coordinate with physicians, social workers, and aides to develop care plans
  • Guide discussions on advance directives and end-of-life choices
  • Proactively manage symptoms and adjust care as needed
  • Maintain accurate, timely documentation in line with hospice regulations
What You Bring
  • Active RN license in Minnesota
  • Hospice or oncology experience (required)
  • Deep compassion and strength to support patients and families during emotional times
  • Clinical autonomy, critical thinking, and great communication skills
  • Experience in an interdisciplinary care model
