Care Partners is seeking Registered Nurses (RNs) and Registered Practical Nurses (RPNs) for positions requiring flexibility in schedules. You will provide nursing care both in-person and virtually, developing effective care plans. The role also includes traveling to various settings such as homes, clinics, and retirement homes.

Key Responsibilities:

• Develop and maintain effective Care Plans for patients

• Deliver exceptional nursing services, in-person and virtually

• Work across various care settings including homes and clinics

• Collaborate with interdisciplinary care teams and family members

• Perform additional clinical duties as required

Requirements:

• RN or RPN degree/diploma with CNO registration

• Strong commitment to improving patient lives

• Valid driver’s license and vehicle for travel

• CPR Certification and immunization records

• Clear background check for patient safety
