Responsibilities
Collect patient data and complete required documentation accurately. Identify patient needs and set priorities, while observing and recording diagnostic results. Use critical thinking skills to make advanced nursing observations.
Develop individualized care plans based on nursing processes, incorporating contributions from other disciplines. Engage patients and families in care planning and coordinate with interdisciplinary support services. Communicate changes in patient status to the team.
Ensure care aligns with accepted nursing standards and organizational policies. Provide treatments, procedures, and other care as prescribed, based on the patient’s needs. Educate patients and families on care plans and 24-hour care delivery. Manage emergency situations and initiate advanced therapy when needed.
Document care provided and any variations, including rationale. Implement nursing measures based on observations and potential problems.
Delegate and supervise direct patient care, ensuring cost-effective and high-quality nursing practices. Participate in quality improvement processes and manage staffing to meet patient care demands.
Contribute to the team, attending meetings and participating in patient care planning discussions.
Other duties as assigned.
QualificationsGraduate of an accredited nursing school.
Active RN license in good standing in the required state.
Minimum of 3 years of recent clinical RN experience.
Experience with electronic documentation preferred.
At least 1 year of experience working with elderly or frail populations is preferred. If not, training will be provided.
Reliable transportation, a valid driver’s license, and the required auto insurance.
Medical clearance for communicable diseases and up-to-date immunizations before direct patient contact.
Successful completion of a comprehensive criminal background check, including verification of federal and state Medicare/Medicaid exclusion lists, criminal history, and other required checks.
Desired QualificationsExperience working with geriatric patients and managing both acute and chronic conditions.
Strong team collaboration skills and effective communication within a team environment.
Basic computer proficiency and familiarity with electronic mail.
Ability to meet competency standards established by regulatory agencies before working independently.
Technical SkillsProficient in EMR systems, Microsoft Office Suite (Word, Excel, PowerPoint, Outlook), and general office equipment.
Strong computer literacy and ability to correspond effectively in writing.
SettingOutpatient Clinic working with the geriatric population, with occasional home visitations as needed.
HoursMonday - Friday and on call
