Responsibilities- Provide 1:1 care for clients following the client’s plan of care at all times while maintaining the client’s dignity and independence
- Administer medications, feedings, oxygen, ostomy care as needed per client’s plan of care and provide assessments of results
- Accurately document nursing actions of all care given and communication with family, case manager, and physicians; document progress and outcomes for established goals and inform physician, case manager, and families of changes in client’s medical condition and needs
- Coordinate or order supplies, medications, and feedings as needed to ensure adequate amounts are always on hand
- Ensure all patient needs are being met in accordance with the instructions of the physician
- LPN/LPT/LVN degree from an accredited college of nursing with current unrestricted registration and license in the state of practice
- Valid driver’s license, acceptable driving record and proof of car insurance
- CPR professional certification
- 1+ years of nursing experience preferred
- Home health care experience preferred

Adoration Home Health / Bright Spring Health Services is an affiliated company that employs those individuals working for Adoration Home Health. The successful candidate will initially be employed by Amedisys until January 1 2026 at the latest. During this period, Amedisys will perform all administrative onboarding activities and offer benefits coverage under Amedisys employee benefit plans. On or before January 1 2026, the successful candidate’s employment will be transferred to Adoration Home Health / Bright Spring Health Services.
The company will make all hiring decisions, supervise and direct the work of the successful candidate.
