Role Specifics and Benefits
  • Lead comprehensive psychosocial assessments for patients and families facing medical illness, identifying barriers to care, gaps in resources, and opportunities for advance care planning.
  • Develop and implement discharge plans that align with medical goals, payer requirements, and community supports, coordinating with case managers, nurses, physicians, and therapists to ensure smooth transitions.
  • Connect patients with needed services, including financial counseling, transportation, housing referrals, community resources, and caregiver support, while maintaining thorough documentation in electronic health records.
  • Facilitate family meetings, mediate complex conversations, and advocate for patient preferences, cultural considerations, and ethical standards in care decisions.
  • Monitor and reassess psychosocial risk factors, crisis situations, and social determinants of health to adapt plans and prevent rehospitalization.
