Key Responsibilities:

• Deliver patient-centered primary health care services

• Promote health education and preventative measures

• Work collaboratively with healthcare teams for optimal care

• Be available for after-hours patient follow-ups

• Maintain thorough and accurate patient records

Requirements:

• Registered Nurse Practitioner with the College of Nurses of Ontario

• 2 years clinical experience preferred

• BScN required; MScN is an asset

• Completion of NP Primary Health Care Program

• Own transportation needed for site travel
