Position: Case Manager with Community Focus
Become a pivotal part of the Canadian Mental Health Association as a Transitional Case Manager in Oxford, Ontario. This full-time contract role emphasizes supporting clients with mental health and addiction needs in achieving recovery goals.

In this full-time, 12-month contract position, you will collaborate with individuals experiencing mental health challenges. Your core responsibilities will involve client assessment, goal setting, and implementation of individualized rehabilitation plans.

Key Responsibilities:

• Conduct various client assessments for needs and barriers

• Assist clients in setting and achieving personal goals

• Develop and implement individualized rehabilitation plans

• Provide education on mental health and available resources

• Coordinate crisis management and support services

Requirements:

• Diploma or degree in health or social services discipline

• Valid G class driver’s license and access to a vehicle

• Satisfactory Police Vulnerable Sector Check

• Ability to regularly attend work

• Experience with serious mental illnesses preferred
