Waypoint is hiring a dedicated Adult Case Manager in Western Maine, offering a flexible hybrid work environment post-onboarding. Engage fully in supporting individuals with disabilities and their families.
In this role, you'll work alongside a multi-disciplinary team to create individualized care plans and ensure the implementation of services for adults with intellectual disabilities. You'll advocate for client needs while managing a smaller caseload, enabling you to provide meaningful support and monitor service effectiveness.
Key Responsibilities:
• Identify client needs and develop tailored care plans
• Coordinate and advocate for identified services
• Track progress and assess service quality
• Collaborate with the multidisciplinary team for optimal outcomes
• Offer guidance for social work licensure support
Requirements:
• BA/BS in human services or similar discipline
• Minimum 1 year experience in the IDD sector
• Must have a reliable vehicle with appropriate insurance
• Valid driver’s license with a clean record required
• Strong English communication skills necessary
