Are you looking for a new senior Learning Disability nurse opportunity?
We're seeking a
Lead Community Learning Disability Nurse to join our Learning Disabilities community team on a 12 month fixed term contract.
You will work as part of this multi-disciplinary team that offers a whole systems approach, which includes:
- Supporting the Team Manager, offering specialist Learning Disability Clinical Supervision, Consultation, joint working and support to the other specialist clinicians and the wider team. Deputising in their absence, ensuring that the complex needs of people with a learning disability are recognised and met appropriately.
- Contributing to the development of a service that is committed to providing high stands of clinical excellence to people who have a learning disability.
- Developing robust networks, ensuring there is excellent multi-agency working processes in place.
- Supporting partner agencies to develop their skills by offering Consultation around adaptions required for people with a learning disability whilst contributing to the development and organisation of team education and training programmes.
- Timely assessments and interventions appropriate to the needs of people and their families, carers, social care, the wider community services and other agencies working with this group across a variety of settings.
- Care Coordinator responsibility for a case load of patients and the delivery of a range of interventions.
You will report directly to the Team Manger and will be responsible for providing leadership and management to nursing and healthcare support staff within their designated area of work at Oxford Health Foundation Trust. In collaboration with the multidisciplinary team (MDT), they will contribute to care delivery, taking a lead on specific areas.
As a Lead Community Learning Disability Nurse, the role involves working as part of the MDT and taking a lead role in nursing team. Providing Clinical Supervision for Band 6 Learning Disability Practitioners. Demonstrate through role modelling high standards of clinical care and the risk management of complex LD cases. To undertake and participate in Audit and Clinical Evaluation of the team working as per Trust guidelines.
