An exciting opportunity for a Band 6 specialist nurse in the Oxfordshire Children’s Diabetes Service. The service cares for 370 children and young people with diabetes across Oxfordshire, providing high‑quality, individualised management. The multidisciplinary team is based at the Oxford Children’s Hospital – John Radcliffe site – with clinics at Alma Road and at Horton Hospital, Banbury. Around 70 % of children use insulin pumps and 90 % of families use continuous glucose monitoring.
Key Responsibilities- Provide specialist nursing advice and education to a caseload of children and young people of all ages, acting as a named nurse.
- Deliver education activities from diagnosis onward, including patient ward management, education at diagnosis, follow‑up multidisciplinary clinics, home visits, school visits, group sessions, and one‑to‑one ongoing education.
- Train ward staff in all aspects of diabetes management and teach student nurses from Oxford Brookes University.
- Use clinical judgement to decide on investigations, management plans, and referrals to specialists.
- Assess families and communities holistically, using physical assessment, history taking, diagnostic tests and developmental assessments.
- Ensure continuity of care and evaluate the effectiveness of treatments with patients and/or carers, making changes as required.
- Work autonomously within a wider health‑care team, serving as a lone practitioner when needed.
- Develop and sustain new partnerships across professional, organisational, and system boundaries, including schools, nurseries, early‑years settings, community services, and parent groups.
- Make evidence‑based therapeutic decisions, including prescribing or adjusting medication, and plan, prioritise, and manage complete episodes of care.
- Use professional judgement in complex and unpredictable care events and apply multi‑agency resources.
- Define practice boundaries appropriately and promote evidence‑based care and policy implementation.
- Communicate effectively, sharing sensitive and complex information with individuals, groups, and agencies.
- Maintain patient documentation to NMC requirements and contribute to clinical data collection.
- Travel to patients’ homes and schools to share expertise.
- Band 6 specialist nurse with appropriate UK professional registration.
- Experience working with children and young people with diabetes.
- Undertaken continuing professional development (CPD) in diabetes.
- Previous experience as a PDSN (Preferred).
- Post‑graduate study or qualification in diabetes (Preferred).
- Experience in an NHS improvement project (Preferred).
- Experience providing diabetes training in school settings (Preferred).
