Community Mental Health Nurse - Assertive Outreach

Oxford Health NHS Foundation Trust invites applications for a Community Mental Health Nurse role within the newly established Assertive Outreach Team (AOT). This innovative service will deliver high-quality, person-centred care across Oxford City to individuals with complex mental health needs, supporting them in community settings, homes, or Trust premises.

Key responsibilities
:

• Act as an autonomous practitioner and active participant in the multidisciplinary team, carrying out specialist nursing assessments and presenting findings to the MDT.
• Partner with patients and their families/carers to develop treatment plans that promote safety, well‑being, independence and community inclusion.
• Implement evidence‑based interventions following NICE guidelines to promote recovery and relapse prevention in varied settings.
• Regularly review care and outcomes, support service users with clinical outcome measures, monitor progress and adapt treatment plans as required.

Essential skills and experience
:

• Registered Nurse with professional registration (RMN) and a background in community mental health or related specialist nursing care.
• Experience working within multidisciplinary teams and managing complex and dynamic risk.
• Knowledge of risk management, assessment and planning frameworks, and evidence-based practice.
• Experience supervising staff or mentoring students, audit work, and using clinical outcome measures.
• Strong communication skills, both written and verbal, and IT literacy.
• Full valid driving licence with access to vehicle for work.

Educational requirements
:

• Degree or equivalent (Professional Qualification).
• CPD portfolio and Post‑graduate education relating to client group, community care or research awareness.
