Location: West Oxfordshire

Our client is seeking an experienced Social Worker to join their West Locality Team in Oxfordshire. This is an excellent opportunity to work within a supportive Adult Social Care service, helping adults across West Oxfordshire to maintain independence, enhance wellbeing, and remain safe within their communities through strengths-based interventions and person-centred support.

As a valued member of the team, you will manage a varied caseload and participate in a duty rota that provides responsive support to urgent situations. You will undertake assessments, safeguarding enquiries, care planning, and reviews, while working closely with individuals, carers, families, health professionals, and partner agencies to deliver positive outcomes and promote community-based solutions.

What will your responsibilities be?
  • Undertaking strengths-based Care Act assessments, reviews, and interventions for adults with care and support needs.
  • Supporting carers through assessment and intervention, ensuring their needs and wellbeing are appropriately considered.
  • Participating in the team duty rota, responding to urgent situations and providing timely advice, support, and intervention.
  • Developing, implementing, and reviewing person-centred Care and Support Plans that promote independence and wellbeing.
  • Applying the Mental Capacity Act 2005 in practice, including Mental Capacity Assessments and Best Interest decision-making.
  • Undertaking safeguarding enquiries in line with the Care Act 2014 and the principles of Making Safeguarding Personal.
  • Assessing and managing risk effectively, promoting positive risk-taking and safe independence.
  • Working collaboratively with health professionals, housing providers, families, carers, community organisations, and partner agencies.
  • Carrying out face-to-face visits, community-based assessments, and telephone interventions.
  • Identifying and facilitating access to community resources, reablement services, equipment, adaptations, and assistive technology.
  • Maintaining accurate and professionally defensible records, reports, assessments, and case notes.
  • Contributing to reflective practice, supervision, and continuous service improvement initiatives.

Qualifications and

Experience:
  • MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.
  • Registered with Social Work England.
  • Significant post-qualified experience within Adult Social Care.
  • Strong knowledge of the Care Act 2014, Mental Capacity Act 2005, Mental Health Act 1983, safeguarding legislation, and statutory guidance.
  • Experience undertaking strengths-based assessments, safeguarding enquiries, reviews, and support planning.
  • Proven ability to work effectively with health services and partner agencies.
  • Excellent communication, assessment, and report-writing skills.
  • Commitment to strengths-based practice, professional ethics, equality, and continuous professional development.
  • Full UK driving licence and access to a vehicle is desirable due to the community-based nature of the role.
