We are seeking a skilled and compassionate Physical Therapist to join a Skilled Nursing Facility in Oxnard, California. This role involves working with a geriatric and post-acute population, helping patients recover from surgery, illness, or injury while improving mobility and overall function. • Evaluate, diagnose, and treat patients with physical impairments in a Skilled Nursing Facility setting
• Develop and implement individualized treatment plans based on patient needs and clinical findings
• Supervise and collaborate with Physical Therapist Assistants to ensure effective care delivery
• Monitor patient progress and modify treatment plans as needed
• Educate patients and caregivers on mobility, safety, and home exercise programs
• Maintain accurate and timely documentation in compliance with facility and regulatory standards

• Active state license as a Physical Therapist in California
• Current BLS/CPR certification
• Minimum of 1 year of recent PT experience preferred, ideally in a SNF or geriatric setting
