, we believe exceptional care starts with strong leadership. We’re looking for a Clinical Director to guide our home health team and ensure that every patient receives the highest quality of care, delivered with compassion, clarity, and commitment.

As Clinical Director
, you’ll oversee patient care services, support our skilled clinical team, and collaborate closely with each department to maintain excellence in care and compliance. This role offers the opportunity to make a meaningful impact on both patient outcomes and staff growth in a supportive, locally owned environment.

• Oversee all patient care services and clinical staff
• Coordinate patient care, assignments, and referrals
• Ensure individualized plans of care are developed and updated
• Provide clinical oversight and leadership to maintain quality and efficiency
• Support agency performance improvement and clinical outcomes initiatives
• Participate in team meetings, supervisory visits, and care conferences
• Manage daily huddles and clinical operations in collaboration with the Executive Director

• Current RN license (required)
• Minimum of one year of recent clinical or management experience (within the last three years)
• Strong clinical judgment, communication, and leadership skills
• Excellent organizational and time management skills
• Proficiency with electronic documentation systems (OASIS experience preferred)
• Home health experience strongly preferred
