Physical Therapist‑PRN
Concept Rehab, Inc. (CRI) invites a physical therapist to provide competent, courteous, and respectful evaluation and treatment of patients with mobility, function, strength, and positioning challenges. The PT collaborates with families, physicians, and other health team members and maintains professional documentation in the medical record.

Responsibilities- Evaluate patients for rehabilitation services within 24–48 hours of physician orders.
- Develop and implement effective treatment plans.
- Supervise licensed physical therapy assistants in clinical service delivery.
- Communicate patient progress, problems, and plans with physicians, clinical managers, and the interdisciplinary team.
- Participate in Plan of Care and interdisciplinary team meetings.
- Meet or exceed the minimum productivity standard established by CRI.
- Conduct in‑service training programs for facility staff.
- Record objective treatment and progress notes, provide input for discharge summaries, and complete daily billing per policy.
- Educate patients’ families and facility staff in maintenance programs and discharge planning.
- Recommend equipment to facilitate improved clinical service delivery.
- Report equipment issues to facility personnel.
- Serve as clinical supervisor for affiliating PT and PTA programs at local universities and colleges.
- Consistently portray CRI’s mission, vision, core values, and professional image.

- B.S., M.S., or D.P.T. in Physical Therapy
- Licensed as a Physical Therapist in the state(s) where services will be provided
