The role involves visiting patients in private homes, skilled nursing facilities, and hospitals, conducting psychosocial assessments, and coordinating community resources.
The ideal candidate will have a CSWE-accredited MSW (Bachelor's considered), be flexible with hours (Mon-Fri, 8am-5pm) and able to document in EMR.
